Read this before you sign the consent form at the shop. It is the plain-language version of what the form covers. The form you sign is the one that counts.

You must be 18 or older
We tattoo adults 18 and older. No exceptions, including with a parent present.
Government-issued and unexpired: driver license, state ID, passport, or military ID.
Utah rules require us to verify your name and date of birth from that ID and keep a signed consent record on file (Utah Code 76-9-2002; Utah Admin. Code R392-701).
No ID, no tattoo. Photos of an ID do not count.
What you are agreeing to
That you are 18 or older and the ID you show is yours.
That you have told your artist about anything that could affect the tattoo or your healing: medical conditions, allergies, skin conditions, medications, pregnancy or nursing.
That you are sober. We will not tattoo anyone who appears to be under the influence of alcohol or drugs.
That you have looked at the design, its placement and the spelling of any text, and approve it. Tattoos are permanent.
That you will follow the aftercare instructions. How the tattoo heals depends on that. See the aftercare sheet.
Your information
Your signed consent form and the ID details on it stay on file at the shop, as the rules require. We do not sell or share them.
Ask your artist before any photo of your tattoo is posted. You can say no.
